To proceed with your Membership Application, you need to download the Application Form. This is downloadable in Adobe Acrobat format (Acrobat reader is required). Please print the form, complete it, sign and return with the membership fee.
The fee required is determined by the application type.

- Family - £15.00
- Organisation - £25.00
Please return your completed Application, along with a cheque made payable to
"North East Help Link Trust Limited"
to cover the membership fee (detailed above).
